Closely partnering with our discharge coordinators, you will ensure that admissions and discharges are seamless, and all clinical assessments are completed within 24 hours of referrals, to minimise bed blocking within local hospitals. In particular, you will make certain that referred patients are suitable for our rehabilitation programme and ensure that colleagues at the homes are fully briefed on their needs. At the same time, you will oversee the implementation of care plans, making certain that we deliver the highest standards of clinical support and outstanding patient-centred services.
Skills & Experience
Experienced in continual assessment within hospital and care settings, you will be fully familiar with nursing tools such as Waterlow and MUST. You will be skilled in writing care plans, capable of supervising colleagues and ready to reshape working practices. Finally, familiarity with dementia care would be beneficial, as would a background within a rehabilitation and step down facility.
Are you an NMC Registered Nurse with experience of working with older people? Do you share our passion for transforming lives? Then you can play a central part in the management of two homes for one of Britain’s leaders in care services.
